Create your account, request your new patient appointment date and time, and you will receive a link to the ‘New Patient Intake’ form automatically.
After you fill out the secure form, click Submit to transmit your new patient information directly into our system. Doc Charlie will review your information, ready for your initial visit. We will also check your insurance and other information and you will be ready to go!
